That Astarion still got erections, perhaps it had something to do with their biology, having a sort of faux-circulatory system so that they could seduce targets. Or it had something to do with the magic that seemed to exist within every living thing on this world.
"I'm going to want to do a more in-depth exam later, in my lab," The doctor stated. "For now, perhaps you could... pour some blood into this?" She held out a small vial, holding it beneath Astarion's hand.
